This interactive session discusses key considerations to coordinate and select on-site dialysis options for your patients. Learn about various operational efficiencies that may be enabled by to transitioning patients from off-site dialysis to on-site dialysis. By keeping patients on-site for treatment, their hemodialysis treatments can be tailored to meet their unique clinical needs while helping to maintain their medication, rehabilitation, and dietary schedules. Best practices for transitions of care will be applied including guiding more patients to home dialysis post-discharge.
