Optimizing Dialysis Care: Transitions From Peritoneal Dialysis to Hemodialysis
Description
The webinar provides an overview of home dialysis utilization trends, the clinical factors that drive transitions between peritoneal dialysis (PD) and hemodialysis (HD), and evidence-based strategies for optimizing patient outcomes during modality changes. The course also introduces the integrated home dialysis model—a patient-centered framework that positions PD and home hemodialysis (HHD) as complementary therapies within a coordinated care pathway.
Upon completion, learners will be able to:
- Review current utilization and trends of home dialysis modalities,
- Understand the causes and timing of PD transitions,
- Identify transition pathways from PD to HD and associated outcomes, and
- Describe an integrated approach model for transitioning from PD to home HD
